The Westchester County Clerk will bring its community outreach mobile office to Mount Vernon City Hall to process passport applications on Thursday, August 28, 2014 from 11 am – 2 pm. Community outreach staffers will be on hand to assist residents with applying for United States passports, including having a photo taken.
To make the process move faster, applicants can fill out Application for Passport, Form DS-11 (downloadable at westchesterclerk.com) that must be signed in front of a staff member. Bring along a Certified Transcript of Birth with raised seal (commonly referred to as long form Birth Certificate that includes parents’ names), valid state or government issued identification such as a valid driver’s license, government identification card or military identification card and a passport photo if you have one.
It will cost $135 for adult applicants (16 and over) and $105 for child applicants (under 16) and can be paid by check, money order or credit card. This amount includes a $25 processing fee.
Please Note: You will need to have a passport to enter the United States by air, land or by sea unless you are a child under the age of 16 entering by land or sea with proof of U.S. citizenship or an adult with another travel document approved by the Department of Homeland Security. All children, including newborn infants, appear with the parent when applying for a passport. Parents of children under age 16 must provide consent of both parents in order for a passport to be issued if one parent makes the application.
Applicants age 16 through 17 must sign their name on the application. Minors involved in custody disputes may not apply for passports if a child’s name has been entered in The Children’s Passport Issuance Alert.
